I think long-haul international flights don’t always go out with full premium cabins, and occasionally odd-timed domestic flights that aren’t going to or coming from hubs/premium destinations can go out with an empty seat or two, but its very rare. 

It’s by no means guaranteed. I’m a Diamond member on Delta (their highest status level) and I still only get upgraded about 50-75% of the time.

Almost all US airlines with a first class section already do this on domestic flights for frequent fliers with status (this is also why first class cabins rarely have an empty seat).

You can rent it from Amazon/iTunes. It’s worth the $3-4 that costs for the soundtrack alone.
