I agree that on its own the car seat issue is unlikely to get CPS to come out. But it should be called in anyway because there may be history with the family or other similar reports and this call could push it to the level that will get a response. I would definitely mention the yelling, too, but that is also likely

I kept my name and we gave our daughter her own last name, so we are all three different. I've never had any negative repercussions from it - no school problems, doctor problems, travel problems, etc. She is 17 now and well aware that it's not the norm, but doesn't give it a second thought. I have recently heard the usage in #2 from sporty types and have also heard it extrapolated to refer to test/exam situations (sort of like #1), but if you are asking which is the original meaning, I would say #4 because I remember that from childhood.

This is what worked (for me at least): superfeet inserts + walking for exercise (instead of trying to jog) + sugarfree diet (and associated 10 pound weight loss) + 6 months = no more foot pain. It was really bad, too. Completely better now.
