The fact that you beat your friends with items because you likely play often with them and know how to exploit some of their quirks does not count as irrefutable evidence that items would make for fair and balanced competitive matches.
If your game has a big shiny heart capable of spawning right in your face that fully heals your character it is not a competitive game. Your skill in the game can still show when you play with items on but the random element means you could still get Mario Kart blue-shelled at any point.
