It doesn't seem like anyone else has mentioned this, but that "bolt" looks like one of those plastic fasteners that help hold all the black plastic pieces together. I would surmise that this one came from said black plastic stuff under the front bumper cover.

My first thought with that quote is that there was an engine failure - perhaps the pilot was pulling up on the collective (to increase pitch on the rotors) but wasn't getting the increased power from the engine to go with it.

Running out of gas on the Lincoln Tunnel helix, shortly after LEAVING NYC, so there was no coasting option. Had to be pushed up the helix to the nearest gas station. I win.

Interesting point. Next to the Sienna or Odyssey, this is the most-popular vehicle in my town. And you kinda described many of the Moms in this town perfectly.
