I think last episode's nightmare hellscape felt notable for being so far beyond what I was expecting from this show. It just kept raising the bar, constantly topping itself in just shoving the weirdest shit imaginable into a 22 minute episode.

So at the end, the entire human race is infected, and the Doctor doesn't seem to know or care.

I'm not sure we're supposed to take that "You said that the last fifteen times" line as anything more than a joke.
