Simpler explanation: the Sideways Alternate Universe isn't really a true Alternate Universe. Hence, trying to figure out where it diverges from the main timeline is kind of pointless.
How/why would Desmond in either of the universes know that Locke A) was paralyzed before the crash or B) missing a kidney? Locke didn't exactly broadcast that fact. Did anyone besides Rose know about the paralysis? And did any of the other survivors know anything about the kidney?

I don't know that I could put MGS2 over MGS3 (maybe my favorite game ever), but I love MGS2 — especially its storyline.
