Having worked in a hospital, I have to agree that a lot of ER services are wasted by those who have non-threatening illnesses but do not want to wait in urgent care. Then everything is cost-shifted to private insurance.

Seasons of The Middle have hovered around 60th place for the year. All the NBC sitcoms you referenced have ratings usually reserved for the CW. 30 Rock, despite the Emmy hit, never got higher than 69th. So, indeed The Middle has a greater audience share due largely to the fact that no one watches NBC outside of sports.
