I think it was the fact that Caroline clearly implied they were sleeping together that made him so batshit crazy. As much as Stefan has been annoying me this season from his - as you say - unearned moral high ground and weird paternalistic desire to "fix" Elena, that scene kind of broke my heart for him a little.
