I agree not completely premeditated, except Walt did remove the gun from the bag and put it in his car. I thought they tried to play it as a murderous rage, since Mike refused to recognize Walt as a great mastermind and basically insulted his ego.

Question and potential plot hole— if the lawyer took the entire $5 million to the bank the first time, which would seem to be the case, since he completely filled Mike's safe deposit box, then why was he back at the bank later with another suitcase full of cash when he was busted? Seems like the writers overlooked

Good point, but is it because Skylar and Hank have changed, or is it because they look much better compared to Walt now? Judging how sullen and irrational Skylar has become, I tend toward the latter.

I agree that Walt's descent has made it less likable, but not less engaging. It has me on the edge of my seat. Each episode. Each scene. The foreshadowing, the performances, the subtle hints. It really cannot be beat on tv today.

This was as appealing as letting Jane die in her vomit. Walt kills Mike because Mike bruised his ego by saying killing Fring was a terrible mistake. Walt flies, into a rage, goes back to his car, and gets the gun. If Walt does not get caught or killed because of his ego, this show will have betrayed what they have been
