The only time I experienced guilt in an open-world title was in United Front Games' excellent Sleeping Dogs. The game was so good at immersing me in the character of Wei Shen, a good man pretending to be a very bad guy, that I felt bad when I accidentally killed a civilian while driving around Hong Kong.
