I don't think it *can* be a Flesh Doctor, because after he's shot the first time he starts regenerating. The Flesh Doctor couldn't, since when the Flesh dies it basically turns back into goop. Right?
Yeah, I think that would have been a bit much. Especially since the gist of showing the former companions was for the Doctor to feel guilty.
It wasn't really a throwaway line. Gaiman specifically wrote that into the script to establish that a Time Lord can change genders if he/she really wants to.
That's what I thought as well. If she's been trained to kill The Doctor, then perhaps she knows that finding out his real name is a pretty big deal.
