well a lot of it is volume, there's a lot more space in the books for characters just to interact, you get pages and pages instead of a single 5 minute scene - and when Jaime isn't talking to Brienne in book 3, he's pretty much always talking to Qyburn.

No he was pretty terrified the whole time and it took a long time for him to actually get it up.

They've made a serious mistake in the pacing of the way they've stretched this out, but I think there is a little more thought going into whats being done to Theon than just torture for torture's sake. (Though obviously the torturer is a complete sadist).
