I thought it looked like a swinging lamp (I'm thinking of the lights in the basement in Stoker, for example) and I wondered if it was something significant to Will, like someday we'll find out a light was swinging back and forth over him after he was stabbed or something like that.

Maybe he enjoys manipulating his patients into thinking they need him, even if he makes them uncomfortable.  That seems to be happening with Will, and it certainly happened with Abigail.
