The problem is you are doing the wrong missions. Only the companion quests and the main quest really have any attached story. There are lots of these, though, so it is easy to always be doing interesting quests. Doing the fetch quests is pointless.

By "actually costs" you are buying into the phony US system. What is the "cost" of using an otherwise idle MRI, or speaking with an otherwise idle doctor? Basically zero.
