Oh, it is a failure at all levels, and yeah, it is a failure of the QA department as a whole that it got through.
Here’s the distinction: The primary job of the vast majority of QA testers is to make sure that the game functions according to the game’s design and specifications.
I’d say less “chose not to say anything” and more “too tired and beaten down to bring it up”. QA work is exhausting, and it can really burn someone out who is just trying to do regression tests and making sure everything in the game matches what’s on the sheet.

Aye, totally true. Visuals are important, it’s just not what I personally look to first (especially since, well, I tend to “watch” a lot of stuff while playing a game or working >_> ).
I don’t hold it against anyone that’s still subscribed to Netflix. Most of my family is, a lot of my friends are who are way queerer than I am. It’s a personal choice and all that, and isn’t always a simple one.
Eh, that’s fair. Dragon Age has such an inconsistent style over the three games (DA1: Low fantasy, color palate of mud, blood and shit. DA2: Super-stylized and fancy, all the colors), I get them just going for the standard style, though.

Except you can’t split them apart, really. One of the reasons that itemization in Diablo 3 initially was so crap was because it needed to keep the really fun and great items super rare, so that they’d become more valuable.
