Quick correction, Ultra-Nightmare is unlocked from the start, and it’s as difficult as Nightmare is but if you die once it’s game over. Beating Nightmare beforehand isn’t necessary.

You have to build floors for walls to connect to but you can remove the floors after you build the walls. Leaves a gap between the walls where the floor was though.
