Sounds like it was done knowingly, but not for the reason you might expect. From their Kickstarter page:

While I see the reason for offense, I'm going to hold off on judging this one. By the sound of the review, it sounds entirely possible like the lead character is in that role precisely because it's where she's expected to be. A difference of a game set in a racist, misogynistic environment, vs a racist, misogynistic…

Keep supporting the Vita and the market it has with more titles that cater to its unique tastes instead of moving on. It may never be large but I expect it can have a large attach rate with what it has.

This is pretty much the norm for iOS. And it's hardly Square's fault. Every year Apple updates iOS and every year games designed for an earlier version break.
