Unless you know the local building regulations, the complete design and load calcs of the staging, if the design met those regulations, and whether or not the structure was assembled to designed specs, then you simply cannot make those statements.

Repeating a mistake is always a bad idea. Do I know that it was a mistake? No, I do not, however the number one "explanation" in theatrical rigging accidents is "...but we've always done it that way." (Who says this? The men who investigate the accidents and literally wrote the book on Stage Rigging.)

If the staging company didn't have a Professional Engineer sign off on the design, everyone involved can be brought up on criminal negligence charges. If someone decided to make a change to the design and didn't get the PE's stamp, that person is toast.
