There's supposedly some kind of cockpit or brain at the back of the neck that needs to be disabled for the Titans to go down. Guess what? You don't have to cut it out if you pulp it instead. And I will again point out that the swords they use wouldn't work in the first place.
Mixed bag, as far as I can tell from this screenshot. No horns, but the helmets look more like Skyrim's than real helmets and that axe is making me feel dumb just looking at it. One strike and the blade would just pop right off.
