Yea I have, it's pretty great for competitive play especially since normal versus mode is balanced too much towards the survivors. Personally I have a bit more fun with the unpredictability and dynamics of (semi) random games though.

You have to kill the doctor because you are Joel, you're pretty much playing his story with all his shitty decisions. I felt real bad having to kill the doctor but it's what makes it great as well in my opinion.
