He likely doesn’t have the original cart. He will have the source data ripped from a devkit, presumably its original assembly format. He can then alter it and compile a ROM from that rather than release the original source files in tact.

It just means it’s Mature audience only (AKA 18+). All the normal places that sell games in Japan will still sell Z-rated titles only some stores will keep them on the top shelf so kids can’t reach them.
This is becoming quite common. The Japanese releases of The Evil Within (re-titled as Psycho Break) and Until Dawn were both also a lot less gory than their western counterparts. Capcom toned down GTA V for Japan as well although I think that was necessary in some parts to be fair.
My first time in Japan was in 2009 so I never really knew a Tokyo with less of these things dotted around. However, I have been reliably informed by my far superior half that it did at one time exist.
