I sometimes get the extended warranty. I payed extra for a lifetime bumper to bumper warranty on my Jeep and it has payed for itself many times over. I drive an 11 year old jeep wrangler still with a bumper to bumper warranty. 

Never assume your health insurance will pay for anything when traveling internationally or domestically. There could be hospitals in your neighborhood that are out of network, do you really believe your health insurance will cover something from another country?
