There is one lieutenant that actually changes something in another quest: Roxana. If you manage to recruit her before a side quest involving Barnabas’ nephew, the outcome of the quest is different and you get some extra scenes with her. These are, btw, completely unrelated quests.

I’m pretty sure they spawn even before you know who they are. Theoretically you can go on a murder spree and end up killing a cultist.

Only five, at most, took assets from another game. These are called fangames. Some fangames there actually use completely original assets to make. (Such as that Smash Bros. demake)
