I know from first hand knowledge that repeat sexual assault when you’re a child does not necessarily include actual penetration. And when there is penetration, it doesn’t always result in breaking of the hymen.

The blood was probably him biting the sides of his mouth and/or tongue during the seizure. It’s very common for those who suffer from seizures; personally my mouth has a lot of scarring from doing this.
