From what I’ve seen, it’s nothing but increased hp and damage from enemies. Everything is just a hit sponge now. No overarching AI tweaks.

Asides from a few optional bosses, I’m not sure I ever died once on the main quest.  Harder difficulty is exactly what this game needed.
