I think you have missed the entire point of the salt and tequila. It’s not to add a slight flavor to the drink, but to numb the taste buds with pure NaCl to cut the fusel taste of the tequila.

There are risks with both the UTI itself and with antibiotics. You seem to not enter the latter into the equation. If there is an overwhelming chance of getting well without antibiotics, and a non-negligible risk of complications from antibiotics, my choice will be to avoid it.

So I’ll say, then your best option is insulin therapy. Refused. OK, but your HBA1C of > 10% is going to destroy you much more quickly than any options there.

I’m absolutely happy to have a conversation with a patient who has concerns about these drugs and if they want to wait for a culture and sensitivity to come back before we start empiric treatment, happy to do that as well.
