Historically, the thinking of police nationwide is that most officers will go their entire careers without drawing their weapon. That has changed in recent years (probably in the last 15-20), but the entrenched thinking hasn’t.
NYC has over 30,000. And you have to figure a portion of those roles aren’t uniformed officers, but detectives and desk jobs.

Even when they are, they RARELY use it. NYPD will stand around and wait for EMS to show up. They’ll rarely even apply pressure to a wound. Even though they’re all equiped with first aid equipment including gloves and CPR shields.
The NYPD firearms qualification is pretty easy to pass. And shooting in the controlled environment of a range is different than a high stress situation. In other words, the training is inadequate and should include more Simunition training.
Cost. Bullets and range time cost money. Instructors cost money. A lot of police departments are underfunded. When they get federal money, it is a one-time check. I think I know exactly what happened. Poor trigger discipline. He had his finger on the trigger. When a person gets scared, there is a natural reaction to clench your fists. If you’re holding a gun and your finger is on the trigger, you are going to pull the trigger.
