It sounds like you've been more than accommodating to his situation. Honestly, based on all the information you've given I think you'll save yourself a lot of heartache if you just cut your loses and accept that he's just not that into you.

For me the same would still apply: tell him that the situation as it is, is not working for you. That while you understand his difficult family situation, his rejection of your meet-up suggestions without offering some kind of alternative or time-frame of when this will be resolved, signals that he lost his interest.
I can understand it is uncomfortable, especially realising that the customer had been eavesdropping, but it seems like he tried to be helpful and not make it creepy by buying the tampons there, and not just randomly bringing them forth from his bag.
