No...I don't think we know how it happened. Honestly sometimes a dog is basically a psychopath. Happened to someone I know, a good dog owner, but her dog's aggression issues couldn't be changed and the vet said the dog's brain was just miswired. I think it's a sad situation and accidents happen.
