For me it was halfway through the 2nd book of David Zindell's trilogy, A Requiem for Homo Sapiens, when the protagonist said "Truly?" once too often.  Instead of "Really?" or "Are you sure?" it was always "Truly?"  Always.  Plus, the protag was a milquetoast.
