I can't entirely get on board with the idea that just "old Russian engines" are by themselves the culprit. It's not like Orbital rolled the engines off a boat and stuffed them up into the rocket's bum without any inspection or testing.

No quite the opposite, it is NATO who has forgotten it and is pushing things to a very dangerous level these past few years. Russia is reminding to morons in DC and Brussels to stay away from its lair and territorial grounds.
