how does the pump know when the tank is empty? Wouldn't it keep on running since it doesn't sense pressure building?

nailed it on the head...the unit was a side-by-side GE.

I don't know about bacterial but our fridge at home developed issues when we let the on board filtration go too long without changing the filter...the system sprung a few leaks as well as destroyed a valve solenoid due to the excess pressure from the clogged filter.
