I think you need to rephrase that. The IAAF doesn’t do surgery. Those women chose to have the procedures done in the hope of meeting the new standard. But no one forced them.

You’re almost getting it. The other women are tested - they look for testosterone as part of PED testing. If you have high levels as a woman, you go in for further testing.
