Yeah, it's absolutely a glaring flaw. For me it wasn't enough to significantly detract from the overall experience, but Kojima's inability to move out of a 13 year old's view of the female body has always been a huge issue that can definitely fuck up the tone of the grandiose stories he aspires to tell.
And that really sucks because I love Quiet as a character and a buddy. I love the conceit but hate the execution, basically.
