So maybe this tensioning caused some unexpected moment in the diagonal support…

I think you’re right. My guess is the workplan was either out of order or not followed. They probably removed the temporary supports prior to post-tensioning the bridge deck, so the deck went into tension.

I think this is it. Those black holes in the deck are for post tensioning cables. My hunch is that these were not installed prior to the temporary supports below the deck being removed. So, when the supports were removed the deck went into tension due to self-weight imposed bending. Unreinforced concrete + tension =…
