What I do not understand is how the pregnancy was not detected. Physical changes aside, in my experience in this type of environment, periods are tracked in patients who menstruate. Missed periods would trigger testing & monitoring.

Likely they want to clarify her condition. I suppose it’s similar, though not entirely comparable to say if your child has autism, and someone said they had down syndrome. You would likely correct them.
