I support Anderson Cooper's characterization of Candidate Trump as a fictional character. It explains so much.

It isn't spite. It's alerting an elected official that this issue is important to you as a constituent.

Addendum to your third point: antibiotic-resistance is a looming concern. Pick up the wrong strain of gonorrhea, and there's only one treatment (until it mutates past that as well). Hilarity ensues. Testing's so much less traumatic.
