In terms of, "Have you not left because he threatened you, or restricted your access to money / medication / food / transportation"...or just in any that doesn't sound like "What the fuck is wrong with YOU, why haven't YOU left this guy, what is YOUR deal?"

The correct way to ask this kind of question is more along the lines of "How long has this been going on? Have you thought about leaving/tried to leave previously?". But, like you unfortunately, in my experience it's always a callous "Why are you still with him?"

You mention activity - I wonder if town planning and general environment is a factor? E.g. I gather many towns aren't set up for cyclists or pedestrians; it is assumed that everyone will drive door-to-door etc; and do these areas have public green/open spaces?
