Solid episode. The one thing that spoiled it slightly for me was the photos. They took away all the moral ambiguity of the execution-style killings, and indicated the writers didn't entirely trust the audience.
I don't believe for one second that Fiona would follow through with that threat if Debbie ended up having the baby. She's saying it because she's doing everything she can to make Debbie see sense. She might even believe it, but that's not the same thing as doing it, when push comes to shove.
Fiona is as close to a voice of reason as you will get on this show. She tends to know what's right, but she's unreliable when it comes to doing what's right, and makes plenty of bad decisions.
My memory might be hazy, but I thought she actually enjoyed looking after him.
Debbie is a pregnant teen filled with raging hormones who thought she had ensnared her dreamboat boyfriend surrounded by the ultimate in dysfunctional families. Not surprising she's acting out this way, really.

I like the speech too, but I don't think Debbie is in denial because she seeing the fun side of babies. She's in denial because she thought it was going to cement the bond between her and the love of her life. By the end of the episode it's finally sinking in that the father isn't coming back, but she's not yet ready…

That's what happens when you stretch a show out over nine seasons of 20+ episodes. There is only so far you can stray and keep within the format and framework of the show, which typically means a lot of recycling and resetting.
