From what I've heard, the Sandvich replaces the shotgun, and heals the heavy, hence making him a bit more hardy on his own.

It's easy to complain about Valve's time schedule and how long it takes them to update everything (I myself am trying to wait patiently for the Heavy Update), but within the constraints of a multiplayer online game, I'm really glad that they've slowed everything down so it's more carefully weighed.
