PS3 Media Server streams stuff from your browser? Like Airplay video style? I haven't played with it in a couple years, but it was more like Plex or something. It was for your video library, not something for when you just wanna play something you found now.

Use MediaMonkey, it had 0 problems with my Beatles collection which includes both the UK and US version of all the albums. Also, you have the option to easily override the album name if for whatever reason it fails.

If it makes you feel better, both what you guessed, what I said, and XSS (another possibility) all are used incredibly frequently. None are all that hard for any hacker type. So there's that :)
They wouldn't have to have downloaded anything. It was probably a Drive By Download (aka the browser downloads it for you, you'd never know you were downloading anything).
