I have interoffice mailed confidential and ananamous letters to Sr management with constructive criticism. That actually has worked to make improvements.
This advise is spot on. In a utopia the suggestions from the author are awesome; however they are not value added in reality. If you work in a small industry, or are well known then saying anything in an exit interview is damning and will burn you in spades.
