Not to be too alarmist, but keep an eye on that. One symptom of a mini-stroke is using really creative descriptions when word-recall fails. My grandmother did that for years before anyone realized she was actually. suffering from dementia after probably a decade's worth of mini strokes.

Yes it is, at least for issues regarding sexual heath. Laws vary from state to state, but there are for sure some laws that require clinicians keep findings about sexual health distinct from medical records for teenagers, and exclude them from requests for medical records without permission from the teen.
Its' also where clinicians will try to find out if there is any abuse in the home.

She looks like she's right about 40, so certainly quite youthful but not like preternaturally young-looking or anything.

Yes it is. You are free to say whatever you please. You are not free from the consequences of it, which may include getting fired if you choose to exercise your freedom of speech on your company's corporate letterhead.
That's exactly what "freedom of speech" means — it is a definition of the limits of the Federal government. It is not a definition for how the rest of us need to treat you if you decide to be an asshole.
But there is an expectation of courteous behavior as part of the social contract that we all live under.
