I don't think this has to be the case if you make the access panel nice and large. Done properly, you could have some really great engine access from that bed. Of course, if you have a bed full of turkey offal and you need to change a plug, it's not so hot.

Why not just a cab-over in front, and then move the "front" bed to the back, giving you 1.5 contiguous beds? Sort of like a non-commercial flatbed with a lift-gate. You're totally right that the engine is just taking up footprint space that could be used for bed, but the best use of bed-space is probably to lump it
