A list like this always welcomes complaints of “what about X?” You can never satisfy everyone with the potential candidates. Someone is always left out.

Any college or university is required by federal law to publicly report sexual assaults by year. Therefore, they have every incentive to persuade women to not report.

But colleges and universities don't even do that. They try to avoid the issue and hope they don't get sued. And they do take their chances with doing nothing. 

That’s like having your coworkers vote on whether or not you should be fired because you embezzled company funds or you sexually assaulted the accountant.

I’ve never understood the point of Title IX reporting. Sexual assault is a crime that should be reported to police. The idea of students being on a college’s disciplinary panel for something like this blows by mind, but apparently it’s not uncommon.  

It’s not just misalignment of incentives - we’re talking about a crime where there is often no physical evidence that something more than a consensual encounter occurred.  There’s a reason police forces have specialized departments for dealing with sexual assault, and asking some college administrator to take on that…

Gosh maybe educational bodies shouldn’t be tasked with meting out justice in matters where their incentives could become misaligned?

To this day, at the University of Virginia, cheating is an automatic explusion whereas sexual assault, even when the perpetrator admits it, is a slap on the wrist.
