Those definitely appear to be a fountain shooting up from the ground (also giving a nice backlight). Placing one of your flashes behind and using a handheld misting fan works for closeups. I’ve also used atmosphere aerosol before to create some volumetric lighting/fog.

If you haven’t tried Mr Gimmick it’s probably one of the best platformers on the NES, and getting all the secret items is a real challenge. That one and Little Samson are probably two of my favorite games for the system
