If the relief valves were incorrectly sized, or if maintenance was lacking, they might have failed to open. Or, if the company changed flow rates or something else critical without revisiting the design of the safety equipment - they may have been correct 15 years ago, not so much now.
Yes. Not to mention alarms and probably other failsafe devices. Relief valves are the last resort thing to open after other stuff goes wrong, and they’re very reliable - I’d be willing to bet that you’re looking at a 1 in 1,000,000 chance of random failure of multiple safeties here. Which probably means someone fucked…
