I'm only very loosely familiar with the Batman TV show. Is there a specific villain they're referencing, or are they just spoofing the fact that outside the Joker, Catwoman, Penguin, and Riddler, they had to go with more and more absurd bad guys?

It was never spoiled for me, but I think it was fairly clear that Omar was going to die. The fact that Omar comes out of "retirement" was a straight-forward enough sign. The major indicator for me, however was the subtle symbolism some episodes later that showed Omar walking uphill going to war with some water
