Actually I just read an article a few hours ago about that specific point where Kit Harrington mentioned that he of all people would LOVE to wear a hat and cover his head but they can't because it's more important to let people see the faces etc. And with the hats it wouldn't be possible.

Looks like you addressed the bombing too - that was one of my issues as well. It felt like it was done for the sake of "action".

More than likely Watson's reaction towards Sherlock was because of the extreme guilt he was feeling for the semi-affair and Mary thinking he was perfect. He was angry at himself and probably taking it out on Sherlock.
Whenever someone says something dumb like "I fell asleep and didn't bother to finish it", yet takes time out of their day to go and comment on a message board about that specific show/episode - I end up blocking them.
Actually I believe Mary mentioned in the episode that she befriended her on purpose, knowing she was the PA.
