it didn't hurt for me! but my betty button is not especially sensitive. if you'd hate a semi-forceful poke in the bellybutton (with a finger) you'd definitely hate it because the ring itself constantly pokes you when you bend over, etc. Plus the healing up time takes for. ev. er.

I dunno, i grew up hearing it as a distinction of, "This person is in the community"/ part of the Minority club, without resorting to describe them through their skin color/race. So i also use it for people who are not black but are a minority and/or empathize very well with black
